Payment Policies

ZANDU HEALTH INITIATIVE

REGISTRATION TERMS AND CONDITIONS 

ARTICLE 1: SPACE IS LIMITED THEREFORE REGISTER EARLY!

Registrations are accepted on a first-come, first-served basis. Spaces for students at the special rate are limited to the grid posted in the course. Please contact us to make arrangements for Offline Payment Plan within five (5) business days of registration. [For Offline Payment: See Article 2: Payment]. 

ARTICLE 2: PAYMENT

Purchase Orders: Third-party Payment agreements must be received before the start date of the course. Please follow up with your employer/ sponsor/ counselor to ensure timely payment of your tuition and fees. 

ARTICLE 3: REFUND POLICY

If your application is rejected, you will receive a full refund of all tuition, fees, and other charges. You will be entitled to a full refund of tuition, fees, and other charges if you give written notice that you are cancelling your enrollment within five days of being accepted into the school or program. This five-day refund policy applies regardless of when the program starts.

If you give written notice more than five days after you have been accepted into the school or program (or first lesson for an online distance education program), you will receive a refund of all tuition, fees, and other charges minus 15%, up to $50, of the total cost of the program. You will be provided a prorated tuition, fees, and other charges refund minus a 25%, up to $100 administrative if you provide written notice of your withdrawal after your program has begun, but before 75% program has completed. 

If you withdraw from your program after 75% of the program has completed, you are not entitled to a refund of tuition, fees, and other charges.You will receive written notice acknowledging your withdraw request within 10 business days after receipt of the notice and you will receive a refund of any tuition, fees, and other charges within 30 business days of receipt of your withdrawal.

Written notice is effective of the date post-marked if sent by mail or the day it has been hand-delivered to the institution. If you do not withdraw in writing or contact the institution about your absence, and you have not attended your program for two consecutive days, you will be considered to have withdrawn from the school as of your last date of attendance.

ARTICLE 4: EVENT CANCELLATION

Zandu Health Initiative (ZHI) reserves the right to cancel or postpone this course if minimum registration requirements are not met or as a result of Force Majeure (events beyond ZHI's control). In the unlikely event of cancellation, ZHI will make all attempts to notify registrants of cancellation or postponement at the earliest opportunity. The registrant will be offered the option to switch to a different course or receive a full refund. 

In case the student cancels, ZHI is not responsible for any costs incurred by registrant(s) due to cancellation or postponement on their part, including but not limited to transportation costs, airfares and hotels. 

ARTICLE 5: SUBSTITUTION OF AN INSTRUCTOR

ZHI reserves the right to substitute course instructors in the unlikely event that instructor(s) listed at the time of registration cannot conduct the courses. Substitution of course instructor(s) is NOT a cause for refund.

ARTICLE 6: CERTIFICATE OF COMPLETION

ZHI provides certificate of completion for each course completed by the registered student.

Please refer to the linked documents before clinical. Steve plans to be at Minnesota Masonic Homes at 6:45am. Clinical begins at 7:00am. In case you are unable to locate your instructor at the clinical site, please text or call Steve (952-208-5590) or Survival (612-524-9881). You may also call the Minnesota Masonic Homes 24 hour desk line at (952-948-7000) for additional guidance.

PARKING

Please park at the Unity Parking lot as indicated on the map here. Please, please drive around to find the Unity Parking entrance. The Dan Patch Parking lot is quite a distance to walk to the Education room (EG12) where we will be meeting.

MEETING WITH EMPLOYEES AT MASONIC HOMES, BLOOMINGTON

When you arrive at Masonic, please follow signs to Unity Entrance. Your goal will be to find the Unity Center facility entrance door. Please feel free to mention that you are a nursing assistant student from ZANDU HEALTH INITIATIVE attending clinical. Alternatively, you can text or call me to update me of your location [Clinical Instructor, Steve Mandieka, 952-479-0172]. Just in case I do not have cell phone signal, please call the Masonic front desk number at 952-948-7000 and request to be directed to where students are meeting for clinical. You may also call or text Survival at 612-298-8044.

When you arrive, please expect to be screened for COVID-19 symptoms at the 24 hour desk, receive one face mask and face shield that you will keep for the entire two clinical days. After screening, each student will wash hands in the 24 hour community room and head down to the classroom on ground floor. Once we get to the classroom and settle in, each student will have a COVID-19 rapid test completed and we will review Student Orientation Paperwork and COVID-19 Infection Control guidelines. CONFIDENTIALITY (HIPAA)

Minnesota Masonic Homes Confidentiality Policy: No cell phone usage (texts/making calls/taking photos) when providing cares to residents/patients. We strictly abide with this policy at CLINICAL. If found using your cell phone, we will enforce this policy in accordance to the School Code of Conduct.

DRESS CODE

  • Eyewear or Face Shield
  • Athletic shoes (sneakers | no open shoes);
  • Transfer belt will be available at the facility (you do not need to bring your own);
  • Please have short nails 1/4-inch-long (no artificial nails);
  • hoodies, fanny packs or sun glasses should NOT be worn when providing care to residents;
  • Shoulder length hair must be secured back away from face.
  • Navy blue uniform
  • Student ID/Name Badge must be visible to residents and staff

MEALS | LUNCH | DINNER

  • Breaks are assigned by your instructor. Usually, a student takes a break at the same time with assigned nursing assistant. Please bring your own boxed lunch or snack. Refrigerator and microwave are available for use. A vending machine with pop and snacks is also available. Alternatively, you may also choose to buy food at nearby restaurants. Use the facility address to search for restaurants

PERSONAL BELONGINGS

Travel light: If you bring any purses/books/skills kit, bags or phone along, please bring a lock to lock your items.

PATIENT/RESIDENT CARE ASSIGNMENTS

Clinical instructor or nurse manager will assign you to shadow a staff member. You will observe followed by active participation in the care of residents. The required outcome at clinical is the student will progress to independence with resident care in the three clinical days. Often, you will work with the same group of residents or the same resident assistant all three days. However, the instructor may modify assignments each clinical day depending on staff availability and student needs.

PLEASE NOTE: You must complete all two days to successfully and the virtual clinical assignment to pass your clinical experience. Plan to be on time to report to clinical. In addition, you may not leave early from the clinical site without instructor permission.
